This notification announces an amendment to the All India Services (Performance Appraisal Report) Rules, 2007. The amendment, known as the All India Services (Performance Appraisal Report) Amendment Rules, 2013, mandates a revised “Form IV” for health check-ups. This updated form includes a comprehensive list of medical investigations and reports, covering haemograms, urine examination, blood sugar, lipid profiles, liver and kidney function tests, cardiac profiles, and specific tests for men and women. It also outlines a section for the “Medical Report of the Officer” and a “Summary of Medical Report” to be attached to the Performance Appraisal Report (PAR). The changes come into effect upon their publication in the Official Gazette.
