अंतर मंत्रालय हॉकी प्रतियोगिता 2018-19 का प्रारंभ दिनांक 26 नवंबर, 2018 से के॰सि॰से॰सां॰क्री॰थाँ॰ के वित्तीय मार्ग खेल परिसर पर होते जा रहा है, मंत्रालयों जो प्रतियोगिता में भाग लेने के लिए रुचि रखते हैं 450 रुपये की फीस के साथ निर्धारित प्रोफार्मा में (पहले से ही परिचालित) केन्द्रीय सिविल सेवा सांस्कृतिक तथा क्रीडा बोर्ड के कार्तिक और प्रशिक्षण विभाग,कमरा न० 361 ‘ब’ विंग, लोक नायक भवन, खान मार्केट, नई दिल्ली-110003 मे जमा करवाये । प्रविष्टि के प्रस्तुत करने के लिए अंतिम तिथि 16 नवंबर, 2018 है एक मंत्रालय से केवल एक ही टीम प्रतियोगिता मे हिस्सा ले सकती है।

This Ministry/Department intends to enter a team for InterMinistry Tournament/Competition. Accordingly the entry form duly filled up is sent herewith. I hereby undertake on behalf of the Ministry to abide by the rules and instruction of the Tournament/Competition.
I certify that the players mentioned in this form who will play in the Inter-Ministry Tournament/Competition are employees of the Central/State Government. No member of the team is a casual / daily wage employee. All the players are eligible to participate in the tournament/competition as per eligibility criteria laid down by the Board . In case if it is found that any member of the team is no eligible and is included in the team, the team will be automatically disqualified from the tournament/competition without any notice to this Ministyr/Department.
I also undertake the responsibility to ensure that this Ministry’s team after being entered in the Tournament/Competition will participate in the Tournament/Competition and at any stage would not refuse to participate or withdraw from the Tournament/Competition without prior approval of the Board.
I also certify that all the players participating in the Inter-Ministry Tournament are physically fit. Although CCSCSB provides first aid, any injury or mishap would not be the responsibility of the Board.
