The Government of India, through the Ministry of Personnel, Public Grievances and Pensions, has issued a notification amending the All India Services (Performance Appraisal Report) Rules, 2007. This amendment, specifically G.S.R. 296 (E), introduces a revised Form IV which is a comprehensive proforma for health check-ups for officers. The updated form includes detailed sections for name, age, sex, brief clinical history, physical and systemic examinations, and a wide range of investigations. These investigations cover haemograms, blood sugar levels, lipid profiles, liver function tests, kidney function tests, cardiac profiles, urine analysis, ECG, X-ray chest, and ultrasound abdomen. The proforma also includes a section for any other relevant investigations and advice. Furthermore, it mandates the submission of a summary of the medical report, including overall health, any remarks, and a health profile grading, to be attached to the Performance Appraisal Report (PAR). This amendment aims to standardize and enhance the health assessment component within the performance appraisal system for All India Services officers.

NOTIFICATION
New Delhi, Dated the16th April, 2007
G.S.R. 296 (E) .- In exercise of the powers conferred by sub-section (1) of section 3 of the All India Services Act, 1951, (61 of 1951), the Central Government, after consultation with the Governments of the States concerned, hereby makes the following rules, to amend the All India Services (Performance Appraisal Report) Rules, 2007, namely: –
- Short title, commencement and application.- (1) These rules may be called the All India Services (Performance Appraisal Report) Amendment Rules, 2007.
(2) They shall come into force on the date of their publication in the Official Gazette. - In the All India Services (Performance Appraisal Report) Rules, 2007, Form IV shall be substituted by the following:-
